"A letter from a (non-relative) dentist, currently practicing or retired from practice in North Carolina." The writer's state is part of the requirement; a dentist outside North Carolina does not satisfy it.

"A letter from the director or supervisor from a community service/volunteer experience," detailing the nature of the work, time served, and skills demonstrated. "Letter should not be authored by a peer or other student." This letter is impossible to earn without real volunteer time, so service is effectively mandatory here even though no hours number is published.

"Enrollment is limited to residents of North Carolina," and every applicant must obtain a Residency Certification Number through the Residency Determination Service before applying. A bachelor's degree from an accredited US or Canadian institution is required. Prerequisites in semester hours: 8 each of biology, general chemistry, organic chemistry, and physics, all with lecture and laboratory for the entire year; 3 of biochemistry, flagged "New for the 2025-2026 application cycle"; 6 of English composition, "ESL courses are not accepted"; 6 of college level mathematics. "A letter grade of C or higher is required for all pre-requisite courses," and everything must be complete by June 30 of the matriculation year. DAT required, no minimum published, and "DAT scores older than three years may not be accepted."
